2.0 Prop. Mactan Water System Improvement Program - Package 5 Ana Construction PhP              38,443,210.45      41,920,833.99 05 Jan 2011 31 Oct 2011 PERT/CPM to be revised - 65.88% - Lapulapu City had revoked the Excavation Permit it issued for the Pajac Road pipelaying works ensuing from the Contractor's failure to complete the  works within the agreed time. Further, the City had informed MCWD that it will not grant Excavation Permits to MCWD Projects.
                  PERT/CPM for revision MCWD had issued a Work Suspension Order to Ana Construction on June 24, 2013. MCWD wrote a letter to the City Mayor on June 25, 2013 expressing apology for the non-completion by the Contractor of the pipelaying works along Pajac Road.and informing the City of the suspension of all works of the Contractor. At present, MCWD is coordinatng with the Mayor's office for a meeting schedule.
                           
3.0 Prop. Ø 300 mm D.I Distribution Line along Pit-os, Talamban, Cebu City New Culion Builders Company PhP              54,099,956.35      53,424,716.82 15 Mar 2012 9 Dec 2012 30 Jun 2013 100.00% 93.94% -6.06% Potability test for the pipeline has failed. This is believed to be due to the wells W34R and W34B supplying the pipeline. This matter is being referred to the PDD.
                          Also, during the hydrotesting of the whole stretch of the pipeline that was conducted by the contractor, an air lock was encountered at the higher elevation of the pipeline particularly the portion near the exit gate of Cebu International School. To address this matter, PMO proposed to put an additional set of Air Vacuum and Air Release (AV/AR) Valve to said portion. PMO requested the approval of the Engineering Department for  the said AV/AR which the latter had approved.

For the additional AV/AR, contractor is requesting for a time extension to cover for the procurement and installation of the valve. The contractor's request is under evaluation by PMO.		 valve, where?, expound

This is in connection with the letter dated July 5, 2013, from the City of Mandaue that was furnished to MCWD instructing the contractor to immediately restore the said area. However, until to date, contractor still failed to complete the restoration.

PMO had ordered the contractor to complete all the remaining works to minimize its liquidated damages.		 slippage?, mention the cause of delay, and the commitment of the contractor.
                           
5.0 Jaclupan Desilting WT Construction PhP                8,000,000.00     17 Jun 2012 17 Dec 2013   30.56% 7.42% -23.14% The Special Waste Disposal Permit of the project had already expired on June 1, 2013. The application for another permit that is good for 90 calendar days is still pending at the Provincial Environment and Natural Resources Office. This is because the newly elected Provincial Governor is requiring a new Mayor's Permit from the newly elected Mayor of Talisay City, and a new Barangay Clearance for year 2013 from Barangay Jaclupan. To date, the total volume of silt that was disposed outside the Jaclupan facility remained at 1,410 cubic meters. Without the new permit, the accumulated volume of silts that were excavated from the ongoing dredging works cannot be disposed outside the facility.  verify when the permit lapsed and what are the activities at site.

6.0 Prop. D.I. Expansion Project at Monterrazas de Cebu Dakay Construction and Development Corporation Php              18,288,888.00   01 Nov 2012 14 May 2013 19 Jul 2013 94.40% 27.80% -66.60% Contractor has already made substantial accomplishments in the pipelaying works. However, some portions are not yet hydrotested.
              PERT/CPM for revision PMO has issued a suspension order to the contractor due to complaints from residents in the area about the muddy road affected by the trench excavation. However, suspension was immediately lifted after contractor has cleaned up the area.
                    The Excavation Permit issued by the Cebu City Engineer's Office limits the work of the contractor during nighttime only (9pm-5am). Consequently, the progress of the pipelaying works is adversely affected. This resulted to the contractor not meeting the project target accomplishment. 
                          Because of the limitation in the working hours, the contractor informed PMO it will request for a time extension. PMO acknowledged the said limitation in the working hours as stipulated in the Excavation Permit. PMO is awaiting contractor's request and then decide on the number of days to grant. The project PERT/CPM will then be revised.
                   
7.0 Proposed Ø250 mm D.I. Water Distribution Line along Sitio Malinao, Agus-Marigondon, Lapulapu City Geo Transport & Construction, Inc. Php 7,488,159.00   28 Jan 2013 01 Jul 2013   99.63% 0.00% -99.63% Lapulapu City had issued the Excavation Permit for the project only this last week of June 2013. Pipelaying works are expected to start early July.
              PERT/CPM for revision. With the release of the Excavation Permit, the project PERT/CPM has to be revised to reflect new targets and schedules. The Contractor can not be faulted for the delayed issuance of the Excavation Permit since the issuance of permit is beyond his control.
                           
8.0 Proposed Ø300 mm D.I. Water Distribution Main  - Canduman, Mandaue City to Tigbao, Talamban, Cebu City Geo Transport & Construction, Inc. Php 17,900,658.75   28 Jan 2013 26 Jul 2013   80.00% 0.00% -80.00% This project involves pipelaying works which fall under the jurisdiction of two local government units, the Cebu City and Mandaue City. Contractor had already secured the Excavation Permit for the Cebu City part of the pipelaying works. However, Barangay Pit-os officials had prevented  the contractor from doing the excavation works.Until to date, the Contractor continues to coordinate with the Barangay officials to get the their permission.
              PERT/CPM for revision The Contractor is still processing the Excavation Permit for the Mandaue City part of the pipelaying works. 
                          The Contractor can not be faulted for the project slippage since the delayed issuance of the Excavation Permit is beyond his control. The Project Targets and Schedules will be revised once an Excavation Permit has been secured where the Contractor can proceed with the pipelaying works.

10.0 Proposed Design and Construction of 1000M3 Modular Water Tank for JV Water Sourcing Program Houston Hydrotech Products and System Inc. Php 10,650,121.15     03 May 2013 29 Nov 2013   1.27% 1.23% -0.04% This is a Design and Build project. As such, the project follows a 2-phase activity: Phase I, the Detailed Engineering and  Phase II, the Construction stage. 
                        For its.work methodology, the Contractor will prepare separately the  Detailed Design for the Site Development and the Modular Tank. The Contractor  had already submitted to PMO the Detailed Plans for the Site Development. It had also submitted the Technical Report and Recommendation on the soil test conducted at the project site. The Contractor had committed to submit the Specifications for the Site Development early July 2013.
                        The Detailed Plans bearing MCWD's comments were returned  to the Contractor for appropriate action. Once the Detailed Plans and the Specifications of the Site Development have been accepted/approved by MCWD, the Contractor will proceed with the processing of the the Buiding Permit.
                        Ending June 2013, the project is generally on schedule.
